What is acne scar?

Acne scars, also known as atrophic scars, are permanent textural changes and indentations that occur on the skin as a result of severe “deep” acne.

How do you get acne scar?
In severe “deep” acne, pus-filled cysts are formed in the deep layers of the skin, which destroys skin tissue (collagen) and “healthy” tissue is not replaced during the healing process. So when the cysts eventually empties and the area heals, it leaves behind an indentation (scar).

Different Types of Treatments for Acne scar?

Treatment 1: Fractional Laser removes the top layers of the skin where the scars are formed. As the top layer of the scar peels off, scars appear less noticeable
Heat and light from the laser also encourages new skin cells to grow, targets blood vessels in scar tissue therefore decreases inflammation.

Treatment 2: Radio Frequency with micro needling, radio frequency energy is passed to the deep layers of the skin through micro needles, which rebuilds collagen and elastin, therefore improves the appearance of scars. “80.64% showed improvement by 2 grades and 19.35% showed improvement by 1 grade.”

Scar Subcision breaks up deeply tethered scar bands associated mainly with atrophic/rolling scars.

Treatment 3: TCA CROSS – TCA (Trichloroacetic Acid) is a chemical peel used to improve acne scarring by placing minute quantities in the scar using a toothpick. CROSS stands for Chemical Reconstruction of Skin Scars.
The most common use of this procedure is for ice pick and small boxcar scars.The process of causing a controlled chemical burn, allows for the body to heal from the deepest part of the scar towards the skin surface.
This makes the scar smaller in diameter and shallower. Usually a series of 3-6 treatments are needed, with 3-4 weeks between treatments.

Different treatments can benefit different types of scars. Rolling scars, recommend radio frequency or laser. Ice pick scars, a combination of laser/RF and scar subcision and TCA.

Laser treatment

Laser treatment works by removing the outermost layer of the skin. With this layer, most of the superficial scarring is also removed greatly reducing the appearance of scars. The laser treatment is a no abrasive approach to removing acne scars and does so without any chemicals or acids. Thus, it is a good option for people with sensitive skin. The laser treatment works well for all skin types and it helps diminish all types of acne scars. Most people with deep or pitted scarring prefer laser treatment as the natural options don’t work as well.

Microneedling

During the Microneedling process, tiny needles are inserted into the skin surrounding the scar and they stimulate them to produce more collagen. The collagen that is produced smooths out the skin and reduces scarring. Microneedling has been known to have a 31% to 62% improvement in the appearance of the scars.

Chemical peel

Many types of chemical peels may work well for acne scarring. A dermatologist will be able to look at your skin type, acne severity, and the type of scarring to determine which chemical peel will work best for you. Chemical peel treatments are known to be extremely effective with a 70% improvement in acne scars. The chemical peels not only work well for scars but also help clear current breakouts and improve the skin’s clarity, tone, and texture.

Acne scar treatment risk? Concerns?

The main concern for Fotona laser (fractional laser/ablative laser) is healing. The laser heats and ablates the most superficial skin layers, there is risk of post inflammatory hyperpigmentation (PIH), PIH is reversible but may take several months.

For microneedling with radio frequency, the only injury to the superficial layers of the skin are needle punctures. There is also swelling, redness and sometimes bruising which subsides within 3-5 days.

For scar subcision, expect some swelling and redness and needle puncture which will subside within 3-5 days.

For TCA (Trichloroacetic Acid) treatments, there will be some mild scabbing for a few days which needs to be kept moistened for best results. Risks include widening of the scar or post inflammatory hyperpigmentation (PIH).
